I John Vitoush do declare on oath that I was born in Bohemia on or about the year 1832 , that I emigrated to the United States of America, and landed at the Port of Chicago on or about the month of June in the year 1867 , that it is bona fide my intention to become a citizen of the United States of America and to renounce forever all allegiance and fidelity to every foreign Prince, Potentate, State or Sovereignty whatsoever, and particularly to Francis Joseph whereof I was a subject.
